if you swing and completely miss the ball (swing over the top of it), is that considered a stoke?

If you were attempting to hit it but missed then yes it is a stroke.

If you swing and intend to hit the ball but miss, it is counted as a stroke. However, if for some reason during your swing, you decide not to hit the ball and swing over the top of it, it is not a stroke. The question of it being a stroke or not is simple: Did you INTEND to hit the ball or not. The difficult part will be explaining it to your playing partners.
